The stomach is an important organ in the body. It plays a vital role in digestion of foods, releases various enzymes and also protects the lower intestine from harmful organisms. The stomach connects to the esophagus above and to the small intestine below. It is intricately related to the pancreas, spleen and liver. The stomach does vary in size but its J shape is constant. The stomach lies in the upper part of the abdomen just below the left rib cage.

Examples including the name gastropathy include portal hypertensive gastropathy and Ménétrier’s disease, also known as “hyperplastic hypersecretory gastropathy”. However, there are many other stomach diseases that don’t include the word “gastropathy” such as gastric or peptic ulcer disease, gastro paresis, and dyspepsia.

Many stomach diseases are associated with infection.

 Historically, it was widely believed that the highly acidic environment of the stomach would keep the stomach immune from infection. However, a large number of studies have indicated that most cases of stomach ulcers, gastritis, and stomach cancer are caused by Helicobacter pylori infection. One of the ways it is able to survive in the stomach involves its unease enzymes which metabolize urea (which is normally secreted into the stomach) to ammonia and carbon dioxide which neutralizes gastric acid and thus prevents its digestion. In recent years, it has been discovered that other Helicobacter bacteria are also capable of colonising the stomach and have been associated with gastritis.

Having too little or no gastric acid is known as hypochlorhydria or achlorhydria respectively and are conditions which can have negative health impacts. Having high levels of gastric acid is called hyperchlorhydria. Many people believe that hyperchlorhydria can cause stomach ulcers. However, recent research indicates that the gastric mucosa which secretes gastric acid is acid-resistant.

Gastritis and stomach cancer can be caused by Helicobacter pylori infection.

There are many types of chronic disorders which affect the stomach. However, since the symptoms are localized to this organ,

The typical symptoms of stomach problems include:

 nausea, vomiting, bloating, cramps, diarrhea and pain.

Your stomach is an organ between your esophagus and small intestine. It is where digestion of protein begins. The stomach has three tasks. It stores swallowed food. It mixes the food with stomach acids. Then it sends the mixture on to the small intestine.

Most people have a problem with their stomach at one time or another. Indigestion and heartburn are common problems. You can relieve some stomach problems with over-the-counter medicines and lifestyle changes, such as avoiding fatty foods or eating more slowly. Other problems like peptic ulcers or GERD require medical attention.

You should see a doctor if you have any of the following:

  • Blood when you have a bowel movement
  • Severe abdominal pain
  • Heartburn not relieved by antacids
  • Unintended weight loss
  • Ongoing vomiting or diarrhea

Disorders of the stomach are very common and induce a significant amount of morbidity and suffering in the population. Data from hospitals indicate that more than 25% of the population suffers from some type of chronic stomach disorder including abdominal pain and indigestion. These symptoms occur for long periods and cause prolonged suffering, time off work and a poor quality of life. Moreover, visits to doctors, expense of investigations and treatment result in many days lost from work and a colossal cost to the financial system.

In the stomach there is a slight balance between acid and the wall lining which is protected by mucus. When this mucus lining is disrupted for whatever reason, signs and symptoms of acidity result. This may result in upper abdominal pain, indigestion, loss of appetite, nausea, vomiting and heartburn. When the condition is allowed to progress, the pain may become continuous; blood may start to leak and be seen in the stools. If the bleeding is rapid and of adequate volume it may even result in vomiting of bright red blood (hematemesis). When the acidity is uncontrolled, it can even cause severe blood loss (anemia) or lead to perforation (hole) in the stomach which is a surgical emergency. In many individuals, the progressive bleeding from an ulcer mixes with the feces and presents as black stools. Presence of blood in stools is often the first sign that there is a problem in the stomach.

Another very common long term problem which is now more appreciated is gastroparesis. Gastroparesis affects millions of individuals and is often never suspected and most patients have a delay in diagnosis. Basically in gastroparesis, the stomach motility disappears and food remains stagnant in the stomach. The most common cause of gastroparesis is diabetes but it can also occur from a blockage at the distal end of stomach, a cancer or a stroke. Symptoms of gastroparesis includes abdominal pain, fullness, bloating, nausea, vomiting after eating food, loss of appetite and feeling of fullness after eating small amounts of food.

During digestion, food is stored in the liquid present in the stomach. The food that is not digested travels to the large intestine and colon in liquid form. These organs begin to absorb the water turning the food into a more solid form. Different viruses or bacteria can increase the amount of liquid that is secreted and moves too quickly through the digestive tract for the water to be absorbed. Diarrhea comes in two types: acute diarrhea and chronic diarrhea. The acute diagnosis can last for a few days up to a week of time. Chronic diarrhea lasts for several days or longer periods of time lasting a few weeks. The difference in diagnosis will help determine the cause of the illness.

Crohn’s disease is an inflammatory bowel disease that can affect any part of the digestive tract, even the stomach, although it’s a rare presentation. Its main feature is inflammatory ulcers that can affect the total thickness of the stomach wall and can bleed but rarely perforate. Symptoms include abdominal pain, loss of appetite, and weight loss. Diarrhea is also a symptom that can develop, so checking stools for the appearance of blood is important. It is possible for symptoms of Crohn’s disease to remain with a person for weeks or go away on their own. Reporting the symptoms to a doctor is recommended to prevent further complications.

Cancers of the stomach are rare and the incidence has been declining worldwide. Stomach cancers usually occur due to fluctuations in acidity level and may present with vague symptoms of abdominal fullness, weight loss and pain. The actual cause of stomach cancer is not known but has been linked to infection with Helicobacter pylori, pernicious anemia, Menetriere’s disease, and nitrogenous preservatives in food.

Smoking has been linked to a variety of disorders of the stomach. Tobacco is known to stimulate acid production and impairs production of the protective mucus. This leads to development of ulcers in the majority of smokers. Chronic stomach problems have also been linked to excess intake of alcohol. It has been shown that alcohol intake can cause stomach ulcer, gastritis and even stomach cancer. Thus, avoidance of smoking and excess alcohol consumption can help prevent the majority of chronic stomach disorders.

One of the most causes of chronic stomach problems is use of medications. Use of aspirin and other non-steroidal anti-inflammatory drugs to treat various pain disorders can damage lining of the stomach and cause ulcers. Other medications like narcotics can interfere with stomach emptying and cause bloating, nausea, or vomiting.

The majority of chronic stomach problems are treated medically. However, there is evidence that a change in life style may help. Even though there is no specific food responsible for causing chronic stomach problems, experts recommend eating a healthy diet which consists of fruits and vegetables. Lean meat should be limited. Moreover, people should keep a diary of foods that cause problems and avoid them.

There are many tools for investigating stomach problems.

 The most common is endoscopy. This procedure is performed as an outpatient and utilizes a small flexible camera. The procedure does require intravenous sedation and takes about 30–45 minutes; the endoscope is inserted via the mouth and can visualize the entire swallowing tube, stomach and duodenum. The procedure also allows the physician to obtain biopsy samples. In many cases of bleeding, the surgeon can use the endoscope to treat the source of bleeding with laser, clips or other injectable drugs.

Other radiological studies frequently used to assess patients with chronic stomach problems include a barium swallow, where a dye is consumed and pictures of the esophagus and stomach are obtained every few minutes. Other tests include a 24-hour pH study, CT scans or MRI etc.

Prophetic medicine is only suitable for good and pure bodies, just as Quran is only suitable for righteous souls and feeling hearts. Therefore, when the people ignore Prophetic medicine, it is like ignoring the help with the guidance of Quran, which is the most effective medicine. Even to this day despite the advances made by science and technology, many mysteries remain. The results and comfort of spiritual therapies comes only from faith and trust in ALLAH. They come through charity, prayer, repentance and seeking ALLAH’s forgiveness. They are aided by doing good deeds, helping the helpless and relieving the afflicted. The causes for these cures are varied and defy logical analysis. And the resultant cures cannot be explained by scientific experimentation and analysis. All this is in accordance of the law of ALLAH and nothing outside it.

When a person’s heart becomes attuned to the Lord of the world, and trusting of the Creator of all ailments and their remedies, medicines which would otherwise be ineffective in an individual who is unbelieving and indifferent of heart become effective,. It has been experienced that when a man’s belief is strong, his body and soul are strengthened and they co-operate in repelling disease and overcoming it. It is most important that the water which people drink and use for other purposes is clean water. This means that the water must be free of germs and chemicals and be clear .Water that is safe for drinking is called potable water.Disease-causing germs and chemicals can find their way into water supplies. When this happens the water becomes polluted or contaminated and when people drink it or come in contact with it in other ways they can become very sick.

Dehydration is a condition that can occur when the loss of body fluids, mostly water, exceeds the amount that is taken in. With dehydration, more water is moving out of individual cells and then out of the body than the amount of water that is taken in through drinking. Medically, dehydration usually means a person has lost enough fluid so that the body begins to lose its ability to function normally and then begins to produce symptoms related to the fluid loss. Although infants and children are at highest risk for dehydration, many adults and especially the elderly have significant risk factors.

People (and animals) lose water every day in the form of water vapor in the breath we exhale, and as water in our sweat, urine, and stool. Along with the water, small amounts of salts or electrolytes are also lost. Our bodies are constantly readjusting the balance between water (and salts or electrolytes) losses with fluid intake. When we lose too much water, our bodies may become out of balance or dehydrated. Most doctors divide dehydration into three stages: 1) mild, 2) moderate and 3) severe. Mild and often even moderate dehydration can be reversed or put back in balance by oral intake of fluids that contain electrolytes (or salts) that are lost during activity. If unrecognized and untreated, some instances of moderate and severe dehydration can lead to death. This article is designed to discuss dehydration in adults.

Thalassemia (thal-uh-SEE-me-uh) is an inherited blood disorder characterized by less hemoglobin and fewer red blood cells in your body than normal. Hemoglobin is the substance in your red blood cells that allows them to carry oxygen. The low hemoglobin and fewer red blood cells of thalassemia may cause anemia, leaving you fatigued.

If you have mild thalassemia, you may not need treatment. But if you have a more severe form of the disorder, you may need regular blood transfusions. You can also take steps on your own to cope with fatigue, such as choosing a healthy diet and exercising regularly.

Facial bone deformities

Slow growth

Abdominal swelling

Dark urine

Several types of thalassemia exist, including alpha-thalassemia, thalassemia intermedia and Cooley anemia. The signs and symptoms you experience depend on the type and severity of your condition. Some babies show signs and symptoms of thalassemia at birth, while others may develop them during the first two years of life. Some people who have only one affected hemoglobin gene don’t experience any thalassemia symptoms.

Causes

Thalassemia is caused by mutations in the DNA of cells that make hemoglobin — the substance in your red blood cells that carries oxygen throughout your body. The mutations associated with thalassemia are passed from parents to children.

Thalassemia disrupts the normal production of hemoglobin and healthy red blood cells. This causes anemia. With anemia, your blood doesn’t have enough red blood cells to carry oxygen to your tissues — leaving you fatigued.

Types of thalassemia

The type of thalassemia you have depends on the number of gene mutations you inherit from your parents and which part of the hemoglobin molecule is affected by the mutations. The more mutated genes, the more severe your thalassemia. Hemoglobin molecules are made of alpha and beta parts that can be affected by mutations.

Alpha-thalassemia                                                                                                         

Four genes are involved in making the alpha hemoglobin chain. You get two from each of your parents. If you inherit:

1 mutated gene, you’ll have no signs or symptoms of thalassemia. But you are a carrier of the disease and can pass it on to your children.

2 mutated genes, your thalassemia signs and symptoms will be mild. This condition may be called alpha-thalassemia trait.

3 mutated genes, your signs and symptoms will be moderate to severe.

4 mutated genes. This type is rare. Affected fetuses have severe anemia and usually are stillborn. Babies born with this condition often die shortly after birth or require lifelong transfusion therapy. In rare cases, a child born with this condition may be treated with transfusions and a stem cell transplant, which is also called a bone marrow transplant.

Thalassemia minor                                                                                                                              

Two genes are involved in making the beta hemoglobin chain. You get one from each of your parents. If you inherit:

1 mutated gene, you’ll have mild signs and symptoms. This condition is called thalassemia minor or beta-thalassemia.

2 mutated genes, your signs and symptoms will be moderate to severe. This condition is called thalassemia major, or Cooley anemia. Babies born with two defective beta hemoglobin genes usually are healthy at birth but develop signs and symptoms within the first two years of life. A milder form, called thalassemia intermedia, also may occur with two mutated genes.

Risk factors

Factors that increase your risk of thalassemia include:

Family history of thalassemia. Thalassemia is passed from parents to children through mutated hemoglobin genes. If you have a family history of thalassemia, you may have an increased risk of the condition.

Certain ancestry. Thalassemia occurs most often in African-Americans and in people of Mediterranean and Southeast Asian ancestry.

Complications

Possible complications of thalassemia include:

Iron overload. People with thalassemia can get too much iron in their bodies, either from the disease or from frequent blood transfusions. Too much iron can result in damage to your heart, liver and endocrine system. This system includes hormone-producing glands that regulate processes throughout your body.

Infection. People with thalassemia have an increased risk of infection. This is especially true if you’ve had your spleen removed.

In cases of severe thalassemia, the following complications can occur:

Bone deformities. Thalassemia can make your bone marrow expand, which causes your bones to widen. This can result in abnormal bone structure, especially in your face and skull. Bone marrow expansion also makes bones thin and brittle, increasing the chance of broken bones.

Enlarged spleen (splenomegaly). The spleen helps your body fight infection and filter unwanted material, such as old or damaged blood cells. Thalassemia is often accompanied by the destruction of a large number of red blood cells. This causes your spleen to enlarge and work harder than normal. Splenomegaly can make anemia worse, and it can reduce the life of transfused red blood cells. If your spleen grows too big, your doctor may suggest surgery to remove it (splenectomy).

Slowed growth rates. Anemia can cause a child’s growth to slow. And thalassemia may cause a delay in puberty.

Heart problems. Heart problems — such as congestive heart failure and abnormal heart rhythms (arrhythmias) — may be associated with severe thalassemia.

Prevention

In most cases, you can’t prevent thalassemia. If you have thalassemia, or if you carry a thalassemia gene, consider talking with a genetic counselor for guidance if you’re thinking of having children.

If you are a physically active you should reduce the risk of heart attack. You should manage your weight better. You should have a lower blood cholesterol level, lower the risk of diabetes and some cancer, lower the risk of blood pressure, have stronger bones, muscle and joint lower risk of developing osteoporosis. Recover better from periods of hospitalization or bed rest. Feel better with more energy, a better mood feel more relaxed and sleep better. It can block your negative thought or distract from daily worries. It provides an opportunity to increase in social contact and boosted your confidence. It also change levels of chemical in your brain such as serotonin, endorphins and stress hormones.

If you are doing regular exercise you should be resist more chronic diseases. Long-term infection with the hepatitis C virus (HCV) is known as chronic hepatitis C. Chronic hepatitis C is usually a “silent” infection for many years, until the virus damages the liver enough to cause the signs and symptoms of liver disease. Among these signs and symptoms are:

  • Bleeding easily
  • Bruising easily
  • Fatigue
  • Poor appetite
  • Yellow discoloration of the skin and eyes (jaundice)
  • Dark-colored urine
  • Itchy skin
  • Fluid buildup in your abdomen (ascites)
  • Swelling in your legs
  • Weight loss
  • Confusion, drowsiness and slurred speech (hepatic encephalopathy)
  • Spider-like blood vessels on your skin (spider angiomas)

Every chronic hepatitis C infection starts with an acute phase. Acute hepatitis C usually goes undiagnosed because it rarely causes symptoms. When signs and symptoms are present, they may include jaundice, along with fatigue, nausea, fever and muscle aches. Acute symptoms appear one to three months after exposure to the virus and last two weeks to three months.

Acute hepatitis C infection doesn’t always become chronic. Some people clear HCV from their bodies after the acute phase, an outcome known as spontaneous viral clearance. In studies of people diagnosed with acute HCV, rates of spontaneous viral clearance have varied from 14 to 50 percent. Acute hepatitis C also responds well to antiviral therapy.

Causes

Hepatitis C infection is caused by the hepatitis C virus. The infection spreads when blood contaminated with the virus enters the bloodstream of an uninfected person.

Globally, HCV exists in several distinct forms, known as genotypes. The most common HCV genotype in North America and Europe is type 1. Type 2 also occurs in the United States and Europe, but is less common than type 1. Both type 1 and type 2 have also spread through much of the world, although other genotypes cause a majority of infections in the Middle East, Asia and Africa.

Although chronic hepatitis C follows a similar course regardless of the genotype of the infecting virus, treatment recommendations vary depending on viral genotype.

Risk factors

Your risk of hepatitis C infection is increased if you:

  • Are a health care worker who has been exposed to infected blood, which may happen if an infected needle pierces your skin
  • Have ever injected or inhaled illicit drugs
  • Have HIV
  • Received a piercing or tattoo in an unclean environment using unsterile equipment
  • Received a blood transfusion or organ transplant before 1992
  • Received clotting factor concentrates before 1987
  • Received hemodialysis treatments for a long period of time
  • Were born to a woman with a hepatitis C infection
  • Were ever in prison
  • Were born between 1945 and 1965, the age group with the highest incidence of hepatitis C infection

Liver cancer

Hepatitis C infection that continues over many years can cause significant complications, such as:

  • Scarring of the liver (cirrhosis). After 20 to 30 years of hepatitis C infection, cirrhosis may occur. Scarring in your liver makes it difficult for your liver to function.
  • Liver cancer. A small number of people with hepatitis C infection may develop liver cancer.
  • Liver failure. Advanced cirrhosis may cause your liver to stop functioning.

Prevention

Protect yourself from hepatitis C infection by taking the following precautions:

  • Stop using illicit drugs, particularly if you inject them. If you use illicit drugs, seek help.
  • Be cautious about body piercing and tattooing. If you choose to undergo piercing or tattooing, look for a reputable shop. Ask questions beforehand about how the equipment is cleaned. Make sure the employees use sterile needles. If employees won’t answer your questions, look for another shop.
  • Practice safer sex. Don’t engage in unprotected sex with multiple partners or with any partner whose health status is uncertain. Sexual transmission between monogamous couples may occur, but the risk is low.

Hepatitis B is a serious liver infection caused by the hepatitis B virus (HBV). For some people, hepatitis B infection becomes chronic, meaning it lasts more than six months. Having chronic hepatitis B increases your risk of developing liver failure, liver cancer or cirrhosis — a condition that permanently scars of the liver.

Most adults with hepatitis B recover fully, even if their signs and symptoms are severe. Infants and children are more likely to develop a chronic (long-lasting) hepatitis B infection.

A vaccine can prevent hepatitis B, but there’s no cure if you have the condition. If you’re infected, taking certain precautions can help prevent spreading the virus to others.

Symptoms

Signs and symptoms of hepatitis B range from mild to severe. They usually appear about one to four months after you’ve been infected, although you could see them as early as two weeks post-infection. Some people, usually young children, may not have any symptoms.

Hepatitis B signs and symptoms may include:

  • Abdominal pain
  • Dark urine
  • Fever
  • Joint pain
  • Loss of appetite
  • Nausea and vomiting
  • Weakness and fatigue
  • Yellowing of your skin and the whites of your eyes (jaundice)

When to see a doctor

If you know you’ve been exposed to hepatitis B, contact your doctor immediately. A preventive treatment may reduce your risk of infection if you receive the treatment within 24 hours of exposure to the virus.

If you think you have signs or symptoms of hepatitis B, contact your doctor.

Causes

  • Mother to child. Pregnant women infected with HBV can pass the virus to their babies during childbirth. However, the newborn can be vaccinated to avoid getting infected in almost all cases. Talk to your doctor about being tested for hepatitis B if you are pregnant or want to become pregnant.

Risk factors

Hepatitis B spreads through contact with blood, semen or other body fluids from an infected person. Your risk of hepatitis B infection increases if you:

  • Have unprotected sex with multiple sex partners or with someone who’s infected with HBV
  • Share needles during IV drug use
  • Are a man who has sex with other men
  • Live with someone who has a chronic HBV infection
  • Are an infant born to an infected mother
  • Have a job that exposes you to human blood
  • Travel to regions with high infection rates of HBV, such as Asia, the Pacific Islands, Africa and Eastern Europe

Complications

Having a chronic HBV infection can lead to serious complications, such as:

  • Scarring of the liver (cirrhosis). The inflammation associated with a hepatitis B infection can lead to extensive liver scarring (cirrhosis), which may impair the liver’s ability to function.
  • Liver cancer. People with chronic hepatitis B infection have an increased risk of liver cancer.
  • Liver failure. Acute liver failure is a condition in which the vital functions of the liver shut down. When that occurs, a liver transplant is necessary to sustain life.
  • Other conditions. People with chronic hepatitis B may develop kidney disease or inflammation of blood vessels.

Prevention

The hepatitis B vaccine is typically given as three or four injections over six months. You can’t get hepatitis B from the vaccine.

The hepatitis B vaccine is recommended for:

  • Newborns
  • Children and adolescents not vaccinated at birth
  • Those who work or live in a center for people who are developmentally disabled
  • People who live with someone who has hepatitis B
  • Health care workers, emergency workers and other people who come into contact with blood
  • Anyone who has a sexually transmitted infection, including HIV
  • Men who have sex with men
  • People who have multiple sexual partners
  • Sexual partners of someone who has hepatitis B
  • People who inject illegal drugs or share needles and syringes
  • People with chronic liver disease
  • People with end-stage kidney disease
  • Travelers planning to go to an area of the world with a high hepatitis B infection rate

Because this immune deficiency is partially compensated by breast-feeding, this is the single best way to protect infants from infection. Malnutrition and nutritional alterations, common complications of human immunodeficiency virus infection, include disorders of food intake, nutrient absorption, and intermediary metabolism and play a significant and independent role in morbidity and mortality. The 21st century provides new information and new challenges. With new technologies and political changes, it is hoped that a healthier, more disease-free, and better-nourished population will emerge.

The 21st century will bring better knowledge of the dynamics and kinetics of specific immune responses; discovery of and better understanding of newer cytokines in the control of nutritional status and immune activation; more real-time noninvasive sampling methods, such as saliva or urine tests for analysis, gene sequencing, and proteomics to examine individual susceptibilities; and improvements in mass vaccination. It may be possible to determine the specific mechanisms by which individual nutrients affect the immune system and thus to directly target activation and regulation of immune pathways.

Controlling malaria, measles, diarrhea, and parasitic infections can also help the body to absorb and retain essential vitamins and minerals. The estimated ratios of benefit to cost for such nutrition interventions range from 4:1 to 520:1

Children, who typically have less nutritional reserve than adults, are particularly susceptible to malnutrition and contaminated water. In addition to potability, water quality is also important for personal sanitation, to prevent contact spread of diarrhea and other infections. Currently, malnutrition is to blame for more than one-half of all deaths of children around the world, primarily deaths caused by diarrhea, pneumonia, malaria, and measles. Poor nourishment leaves children underweight, weakened, and vulnerable to fatal and nonfatal infections. It is estimated that feeding all children worldwide an adequate diet would prevent ∼1 million deaths per year caused by pneumonia, 800,000 caused by diarrhea, 500,000 caused by malaria, and 250,000 caused by measles . These conditions are preventable, providing that the insight, coordination, political will, and funding are available.
